Please note:
  • These policies are only available in the state of NY. Coverage and rates available to members in other states may differ or may not be available.
  • Pre-existing condition: A condition for which medical advice was given or treatment was recommended by, or received from, a licensed health care provider within 12 months before the Effective Date of coverage OR symptoms which would ordinarily cause a prudent person to seek diagnosis, care or treatment within 12 months prior to the Effective Date of coverage.
  • Pre-existing condition limitation: Benefits are not payable for a pre-existing condition within 12 months of the Effective Date of coverage.
  • Waiver of premium benefit: If the primary insured is disabled for at least 90 days and unable to make premium payments, the policy will be paid by the insurance company as long as the primary insured remains disabled.
